How does Labetalol work?
Alpha1 receptor blocker
Nonselective Beta receptor blocker
It lowers blood pressure in a hypertensive crisis. Blood pressure is reduced without reflex tachycardia, and total peripheral resistance is decreased, helping maintain cardiac output. Usually given IV in emergency care.
What is the onset of Labetalol?
within 5 minutes
What is the duration of Labetalol?
3-6 hours
What are indications for Labetalol?
- hypertensive emergencies
- suspected MI and unstable angina
- useful as adjunctive agent with fibrinolytic therapy
- convert to NSR or to slow ventricular response in supraventricular tachydysrhythmias
- reduce myocardial ischemia in AMI patients with elevated HR and BP or both
What are contraindications for Labetalol?
- hemodynamically unstable patients
- STEMI if there is signs of increased risk cardiogenic shock
- 2nd or 3rd degree block, active asthma, reactive airway disease, severe bradycardia or SBP <100mmhg
What are some adverse reactions of Labetalol?
- headache, dizziness, fatigue, flushing, diaphoresis, N/V
- edema
- vertigo
- ventricular dysrhythmias
- dyspnea
- allergic reaction
- dose-related orthostatic hypotension
- bradycardia
- tinnitus
What are some drug interactions with Labetalol?
- bronchodilator effects of beta-adrenergic agonists may be blunted by Labetalol
- Nitro may augment hypotensive effects
How is Labetalol supplied?
5mg/mL in 4-, 8-, 20-, and 40-mL vials
What is the IV dose of Labetalol?
10mg IV over 1-2 minutes
may repeat or double Labetalol every 10 minutes to a max dose of 150mg
What is the dose for Labetalol infusion?
mix 200mg in 250mL of D5W
infuse at 2-8mg/min, titrate to supine BP
What are some special considerations for Labetalol?
- pregnancy safety category C
- observe for signs of CHF, bradycardia and bronchospasm
- Labetalol should only be administered when the patient is supine
