Umbilicus= landmark; go typhoid to cranial pubis

What is the aim of the ventral midline approach?
aim is to incise through linea alba
What is in the right quadrant?
duodenum, kidney (epolic foramen), right ureter,

diaphragm, stomach, liver/gallbladder, pancreas
What is in the left quadrant?
descending colon, left uterine horn, adrenal, spleen
What is central?
cecum, jejunum, mezcenteric LN, transverse colon
What is the caudal quadrant?
ventral ligament of bladder, prostate, uterine body, duodenocolic fold
How many stay sutures are there?
2 lateral and 1 apex
What do you use to extend the stab incision?
Mesentbalms scissors
What’s the holding layer for the bladder?
submuscosa
What suture patterns do you use for the bladder?
first layer continuous; second layer inverting- either cushing or lembert
What is the holding layer for the celiotomy?
External rectus fascia
What’s the suture pattern for the external rectus sheath?
simple continous or simple interrupted
What’s the difference in the closure for a paramedic or a ventral midline?
THEY ARE THE SAME!
what suture do you use for a celiotomy?
monofilament, multifilament, non absorbable
what are the sutures to avoid?
chromic gut, silk, vicryl rapid, polymerized caprolactam
When you close the fascia how big are the bites?
5mm by 5mm
