What are appropriate wide and length ACE wraps for TF and TT

TF — use 6 inch width and 2-3 lengths
TT — use 4 inch width and 2 lengths

What is the wear schedule for ACE wraps

A
  • Should be worn 24 hours a day!!!
  • Remove only for bathing/dressing changes
  • Rewrap immediately
  • Wear during, but rewrap after exercise periods as it could lose some of the compression

When should you re-wrap

A
  • every 4 hours
    OR
  • when compression over the distal end of residual limb is lost

When should you clean the bandages

A

DAILY
- good to have multiple sets of wraps so you can still wrap the limb while the other ones are being cleaned and dried.

Why do we wrap

A
  • Want to provide compression asap !!!
  • Shaping the limb to get ready for prosthesis down the line
  • Compression more distally moving proximally helps with edema
  • Protect scar

When do you stop bandaging

A
  1. When residual limb volume has stabilized (6-8 weeks of all day prosthetic use)
    — If amputee leaves bandage off overnight and donning the prosthetic in the morning is not difficult and the residual limb volume has stabilized then bandaging can be discontinued
    — if there is persistent fluctuation in limb volume may necessitate continued wrapping fro an indefinite period.

When do you reinstate wrapping after initial prosthetic wear?

A
  1. If not wearing the prosthesis daily
  2. Temp discontinuation of prosthesis for any reason

General principles of elastic wrapping of residual limb

A
  1. Pressure should be max over distal end and gradually reduced moving proximally
  2. No horizontal or circular turns as these tend to constrict circulation — always use oblique turns
  3. Never encircle both medial and lateral aspects of distal end in one turn bc causes pucker up on the scar and not good
  4. Cover entire distal end of limb at least 2 times with bandages
  5. Tape is the safest anchor of the bandage. If you use clips or safety pins the end much be on anterior/lateral surfaces
  6. Apply tape only to bandage and not directly on skin!!

A
  1. Supine with hip fully extended
  2. Sidelying on uninvolved side, with residual limb in extension
  3. Sitting on edge of bed or chair for self wrapping
