First thread
Ailing
Failing

Ailing: peri-implant mucositis
Failing: Peri-implantitis, progressive bone loss, pocket, BOP
Failed: Peri-implantitis, hopeless, nonfunctional, mobility

Treatment of failing implants
Resolve inflammation
Correct pseudopockets w/ flap or gingivectomy
Re-osseointegrate w/ GBR
Classifications of peri-implantitis
1: slight horizontal
2: Moderate horizontal w/ isolated vertical
3: Moderate/advanced horizontal w/ circular defects
4: Advanced horizontal, circumferential vertical defects, loss of wall
Class 1 treatment
Surgically reduce pocket depth
Apical reposition
Clean implant surface
Implantoplasty if threads exposed
Class 2 treatment
More apical repositioning than 1
GTR if vertical w/ 3 or more walls
Osteoplasty if 1 or 2 walls
Treatment for vertical vs horizontal defects
Vertical: GTR
Horizontal: implantoplasty
Class 3 and 4 treatment
GTR
Treatment of failed implants
Remove implant
Remove implant, GBR, Place new implant
Remove implant from function, decontaminate and submerge
