Q

Hypothalamic-Pituitary-Adrenal Axis?

A

Hypothalamus:
• Corticotrophin-releasing hormone (CRH)

Pituitary:
• pro-opiomelanocortin (POMC)

Q

Hyperaldosteronism-excessive aldosterone: Primary and secondary?

A

Primary hyperaldosteronism “Conn’s syndrome”
• Adrenal Adenoma
• Lab finding: High aldosterone, low renin

Secondary hyperaldosteronism (high renin)
• Common, associate with renal, heart or liver disease
• Lab finding: High aldosterone, high renin

Q

Hypercortisolism = Cushing’s syndrome?

A

Primary hypercortisolism
• Adrenal tumor

Secondary hypercortisolism
• Pituitary tumors–referred to ‘Cushing’s disease’
• “Ectopic” ACTH production by non-pituitary tumors

Iatrogenic (drug) or exogenous: steroid medication

Q

Diagnosis of Cushing’s Syndrome?

A

Confirmation Test (Dynamic test)
• Dexamethasone suppression tests
• Dexamethasone is an exogenous corticosteroid
• Acts like cortisol
• 2 versions with different utility
• Low dose –confirm Cushing Syndrome diagnosis
- Give drug at midnight, measure cortisol at 8 am the following day
- All etiologies of Cushing’s Syndrome will yield elevated cortisol regardless at this low dose
• High dose –determine etiology
- Give drug at midnight, measure cortisol and ACTH at 8 am the following day
- Dose is potent enough to inhibit pituitary adenoma ACTH

A

also called Addison’s disease or Adrenal Insufficiency
