Who are the board members?

How are board members selected?
the state pharmaceutical association shall submit a list of candidates to the governor and the governor may or may not select members from that list.
How long do Board members serve?
maximum limit of two terms of four years each; may or may not be consecutive.
How often does the board convene?
the board shall meet at least four times per year

They are open to the public
Who can apply for a pharmacist’s license? (New Practitioner)
any person at least 18 years old and graduated from college or school of pharmacy or department of a university recognized by the board
Who can apply for a pharmacist’s license? (Transfer/reciprocity)
Must have met internship requirements from state of reciprocation or have at least one year of practice as a licensed pharmacist
Who can apply for a pharmacist’s license? (Foreign Graduate)
each applicant who has graduated from a school or college of pharmacy or a pharmacy department of a university located outside of the United States.

Reinstatement Process for Pharmacists
Submit form LA-60 to Board office
If not practicing in the last 5 years must take NAPLEX
If more than 3 years since expiration/ cancellation, must re-take and pass MPJE
If one renewal period has lapsed since expiration/ cancellation, must complete 30 hours CE credit
If two or more periods have lapsed since expiration/ cancellation, must complete 60 hours CE
Probation Reasons
- license has obtained, renewed, reinstated, or attempted to obtain, renew, or reinstate, a license by false or fraudulent means
- the licensee has been convicted of a misdemeanor involving moral torpid or gross immorality or any felony and the licensee fans to show that the license has been sufficiently rehabilitated to warrant the public trust
- the licensee is found by the board to be guilty of unprofessional conduct or professional incompetency
- the licensee is addicted to the liquor or drug habit to such a degree as to render the licensee unfit to practice the profession of pharmacy
- the licensee has violated a provision of the federal or state food, drug, and cosmetic act, the federal or state uniform controlled substances act, or any rule or regulation adopted under any such act
- the licensee is found by the board to have filled a prescription not in strict accordance with the directions of the practitioner or a mid level practitioner
- the licensee is found to be mentally or physically incapacitated to such a degree as to render the licensee unfit to practice the profession of pharmacy
- the licensee has violated any of the provisions of the pharmacy act in the state of Kansas or any rule and regulation adopted by the board members pursuant to the provisions of such pharmacy act
- the licensee has failed to comply with the continuing education requirements of the board for license renewal
- the licensee as PIC or consultant pharmacists under provisions has failed to comply with the requirements laid out by KSA 65-1648
- the licensee has knowingly submitted a misleading, deceptive, untrue, or fraudulent misrepresentation on a claim form, bill, or statement
12) the licensee has had a license to practice pharmacy revoked, suspended or limited has been censured or has had other disciplinary action taken
13) the licensee has self-administered any controlled substance without a practitioner’s prescription order or a mid-level practitioners prescription order
14) licensee assisted in suicide
15) the licensee failed t furnish the board any information legally requested
16) the licensee has violated or failed to comply with any lawful order or directive of the board
17) the licensee has violated any of the provisions of the prescription monitoring program act of the state of Kansas or any rule and regulation of the board pursuant to the provisions of the prescription monitoring program act
18) falsified records
Process for probation
pharmacist must wait a minimum of one year from date of revocation before applying for reinstatement
pharmacist must submit form LA-65, S-100, FD-258, and proof of CE
Renewal period of pharmacist license?
