Dehiscence
A partial or total separation of wound layers
Eschar
Hard black, brown, tan or necrotic tissue. They need to be removed before healing occurs.
Evisceration
Protrusion of visceral organs through an opening wound
Exudate
fluid that oozes out or discharge of tissues during inflammation
Friction
The force of two surfaces moving across one another.
Granulation Tissue
Red, moist tissue composed of new blood vessels, progression indicates healing.
Hematoma
Localized collection of blood underneath the tissue. “A bad bruise”
Hemorrhage
Bleeding from a wound site and is normal during and immediately after trauma
Hemostasis
Injured blood vessels constrict and platelets stop bleeding. This controls blood loss, estabish bacterial control, and seal injury.
Induration
hardening of the tissue, (particularly skin) bc of edema or inflammation
Laceration
Torn jagged wound or a deep cut
Negative Pressure Wound Therapy
Application of negative pressure to a wound thru a suction to heal and collect wound fluid.
Nonblanchable Erythema
Erythematous area does not blanch when applying pressure. Reddish- blue color with skin intact surfaces
Pressure Ulcer
Ulcer in the skin over a bony prominence
Primary Intention
Primary union of the edges of the wound, progressing to a complete scar formation w/o granulation
(Wound that is closed)
Puncture
a small circular wound with edges coming together toward the center
Purulent
Thick, yellow, green, tan, or brown drainage (like pus)
Sanguineous
A wound drainage that is bright red and indicates active bleeding
Secondary Intention
Wound edges are NOT closed or approximated.
(Wound closure which edges are separated and granulation develops to fill the gap. This creates a larger scar)
Serosanguineous
A wound drainage that is pale, pink, watery. A mixture of clear + red fluid
Serous
A wound drainage that is clear, watery plasma
Shearing Force
Weight pressing down + friction
Slough
Soft yellow or soft tissue on wound bed
Sutures
Threads or metal used to sew body tissue together
Tissue Ischemia
Tissue receive insufficient oxygen and perfusion
Vacuum-assisted Closure (V.A.C)
device that helps wound closure by applying negative pressure to draw edges of would together
Wound
Disruption of integrity and function of tissues in the body.
