The key signature is the note name a half step above the last sharp - eg F# is the last sharp in the key signature of G - G is half a step above F#
Does an accidental apply to the rest of the bar/measure
Yes.
If an accidental marks a B as flat, all of the B’s in that bar/measure are flat
