Work up
“HHV-8, HIV
Classic=elderly men with Mediterranean or Eastern European heritage
Endemic=sub Saharan Africa
Epidemic= AIDS associated
Iatrogenic=secondary to other immunosurpression”
Treatment
“Localized: surgery or RT
Diffuse: chemo or TSI
Classic or endemic:
Single area: 8-12 Gy single dose, 24 Gy in 12 fx, or 30 Gy in 15 fx
Total skin: 4 Gy weekly for 6-8 weeks
Oral mucosal: 15 Gy in 10 fractions
Epidemic (AIDS related) Kaposi’s: treat AIDS with HAART. RT is never first line. Localized: aliretinoin gel, imiquinoid cream, or intralesional vinblastine.
Systemic treatment with liposomal doxorubicin or paclitaxel. RT typically given in 24 Gy in 12 fx. Other palliative regimens are also appropriate.”
