What are goals of physical and psychological evaluation before sedation?
Determine physical and psychological ability to tolerate treatment.
What is required for physical exam for conscious sedation?
Medical history questionnaire Physical exam Dialogue history
What sedation medications are contraindicated in asthma patients and why?
No barbiturates or opiods, especially demerol -increase likelihood of bronchospasms
What medical history is local blocks contraindicated?
Hemophilia- must avoid blocks in which positive aspiration is at risk (IAN, PSA)
What are the vitals signs needed for conscious sedation?
HR BP Temp RR Ht Wt
FDA pregnancy categories?
A- studies have failed to demonstrate risk to fetus in any trimester B- Animal studies fail to demonstrate risk, no human studies available C- Only given after risk to fetus are considered, no human studies; animal studies demonstrated risk D- Definite human fetal risk, may be given in spite of risk if life threatening conditions X- absolute fecal abnormalities, do not use any time during pregnancy because risk outweigh benefits.
What is auscultatory Gap in BP measuring?
An error in BP readings. A sound reappearing at a lower systolic level. Need to palpate for radial pulse to confirm BP
ASA class 1?
a patient without systemic disease
ASA II
a patient with mild systemic disease
ASA III
a patient with severe systemic disease that limits activity but is not incapacitating
ASA IV
- a patient with incapacitating systemic disease that is a constant threat to life
ASA V
a moribund patient that is not expected to live 24 hours
What are the stress reduction protocols?
Recognition of anxiety pre-medication appointment scheduling adequate pain control duration of treatment postoperative pain management
How do you calculate when a patient is hypotensive?
70 + 2(pt age) = hypotension BP
Average BP ranges
