Mechanical Trx
Debridement, avulsion and laser therapy
Debridement
removal of foreign material and dead or damaged tissue
Avulsion
Removal of all or part of the nail plate
OTC
Formula 3/Proclearz (1% tolnaftate) - mild fungicidal/antifungal
Lotrimin (Clotrimazole 1%) -Antifungal spec for yeast
Oral Antifungal Agents
Highest rates of complete/mycological cure
Primary concern is potential drug interaction and adverse events including liver.
ie. terbinafine (3-4 month therapy)and itraconazole (12wk therapy)
Topical Antifungal Agents
Primary therapy in mild infectiosn
Pallative therapy for those unable to take oral
Prophylactic therapy to help prevent infection
ie Penlac (Ciclopirox) Kerydin (tavaborole)
Lamisil ( Terbinafine)
250mg tablet
1x a day for 12wks
Warnings-Hepatoxicity
Penlac ( Ciclopirox)
Lacquer solution in 6.6ml bottle w/screw caps wit brush
mild to mod infection
daily for 48wks
