What options are available for IVF?
Sterile H20 D5W 1/2 NS Normal Saline 3% Saline
When do you use sterile H20?
Never
hypotonic
When do you use D5W?
Patient is hypernatremic
When do you use Normal Saline?
Patient is hypovolemic
When do you use 3% saline?
Patient has severe symptomatic hyponatremia
When do you use 1/2 saline?
Patient is making dilute urine, can’t concentrate urine
What can you add for patients who are hypokalemic?
KCl
When would you add bicarbonate?
Patient has a metabolic acidosis and is unlikely to make bicarb
What is D5W?
5% dextrose in water
hypotonic but isoosmotic
What is 1/2 NS?
0.45% NaCl
Na+ = 77 mEq/L
Hypotonic
What is normal saline?
0.9% NaCl
Na+ = 154 mEq/L
Isotonic
What is 3% saline?
….3% NaCl….
514 mEq/L
Hypertonic
When would you use hypotonic fluid?
Patients with hypernatremia or tendency to lose water
What would you use for a euvolemic patient having surgery?
Isotonic fluid - normal saline
