Key drugs that have sorption or leaching issues with PVC bags
LATTIN: lorazepam, amiodarone, tacrolimus, taxanes (except Abraxane), insulin, nitroglycerin
Key drugs that should be put into SALINE (no dextrose)
A DIAbetic Can’t Eat Pie: Ampicillin, Daptomycin, Infliximab, Ampicillin/sulbactam, Caspofungin, Ertapenem, Phenytoin
Key drugs that can be put into DEXTROSE (not saline)
BOAS: bactrim, oxaliplatin, amphotericin B, synercid (quinupristin/dalfopristin)
Fatal IV med incompatibilities (2 common ones)
Calcium and Ceftriaxone;
Calcium & Phosphate
*LR contains calcium
Common inpatient IV drugs that are incompatible with most drugs

Most commonly used in-line filter size
0.22 microns for most. 1.2 micron for lipids
Key drugs that need in-line filters
GAL, PLAT: Golimumab, Amiodarone, Lorazepam, Phenytoin, Lipids, Amphotericin B (5 micron), Taxane (except docetaxel)
** only need for continuous infusion
