  1. IV push over 2-3 min.
  2. 2mg for pain 7-10, 1mg for pain 4-6, 0.5mg for pain 1-3.
  3. Don’t dilute (but actually do. Remember 1ml per min. So, if you’re gpoing to push for 3 min, you need 3 ml.)

  2. 1g over 60min
  3. 1g. reconstitute a 1g vial with 20ml sterile water for injection. Then dilute w/ 100ml of 0.9% NaCl, D5W, or LR for every 500 mg of Vancomycin being administered.
