IT Band Syndrome Flashcards
What is the IT band made up of
Fibres from the aponeurosis tensor fasciae latae and glut max
Where is the IT band from and too
from the iliac tubercle to the anterolateral tubercle of the tibia
What does inflammation of the IT band result in
iliotibial band syndrome (ITBS).
What is the pathophysiology of ITBS
thought to be linked to repetitive flexion and extension of the knee, causing impingement of the band against the lateral femoral condyle.
what are the risk factors for ITBS
repetitive flexion and extension of the knee, commonly runners, weightlifters, or cyclists
genu varum, excessive internal tibial torsion, foot pronation, and hip abductor weakness.
What are the clinical features of ITBS
Lateral knee pain - exacerbated by exercise
What are the special tests that can be carried out for ITBS
Nobles test - supine position and the examiner places a finger on the lateral femoral condyle, with the knee slowly extends. A positive test is indicated when pain is felt at 30 degrees, when the ITB passes over the lateral femoral condyle.
Renne test - examiner stands in front of the affected knee and puts pressure on the lateral epicondyle, with the patient then asked to squat. A positive test is indicated by the presence of pain at 30 degrees of flexion.
What is the differential diagnoses
Degenerative joint disease
Fractures
Ligamentous injury
What investigations would you carry out
Diagnosis is made clinically
However can do x rays and MRI imaging to exclude other pathology
What is the non surgical management of ITBS
Modify activity
Simple analgesia
Longer term management may include corticosteroid injections, Physio,
When is surgical management of ITBS indicated
Surgery is only indication if patients remain symptomatic or functionally limited after 6 months, despite all other non-medical treatment.
What does the surgical management of ITBS
release of the iliotibial band from its attachments from the patella
can be done either percutaneously or via an open approach.
