IT and Medical Informatics Flashcards
What is passive vs active IT
Passive = storage of info (dx)
Active = reminders, feedback, prescribing alerts
What are the 3 categories of IT
Administrative/financial
Clinical systems (drug interaction checks patient portals)
infrastructure (handheld devices)
What are the 3 levels of IT interoperability from lowest to highest
- Foundational: just dropped into a box and then someone has to manually add it to a chart (not ideal but MC)
- Structural: exchanging data and some data carries over (goes to patient’s chart)
- Semantic: from one facility to the next and the system collects it (same software)
What are the benefits of IT
improved eased of access, stops duplications
What are the barriers to IT
Cost
Lots of different systems
Complexity
Might not be cost effective actually
No streamlining between facilities
Slower then paper charts actually
What is EHR
Electronic health record
has demographics
communications
encounters
consults w/ providers
hospital records
procedures
diagnostic testing
advanced directives
How is an EHR organized?
Through templates
structured (filled in with boxes)
unstructured (fill in yourself)
What is an electronic medical record
for CLINICIANS
does not have all records, just pertinent one
clinical-related functions
confidential notes/ reminders (be cautious about bringing up her weight for some1 with an eating disorder for instance)
name alert
What is a patient health record?
For PATIENT
can upload BP logs
what is an electronic health record?
ALL components of EMR and overall health information for a given patient
What is computerized provider order entry
CPOE
replaces facility paper-based ordering systems
electronically write full range of orders
can bring up errors
reduces handwriting or transcription
keeps track of clinical decisions
Why was CPOE developed?
To decrease hospital errors
However, in practice, it has not reduced errors :(
What is a clinical decision support system
made for providers
- can offer screening suggestions
- can offer suggestion for treatment and have it prewritten
- can include summaries of specific organ system labs
- can help give you diagnosis
- can be customized
- gives data for how often providers in your hospital follow certain guidelines
What is a Picture Archiving/ Communication System?
PACS
improved organization
Digital films
Very costly
What is bar coding used for
Just identifies things and does not prevent using the wrong med
just scan and keep track of things
What is a radiofrequency identification? (RIFD)
Tags to identify people/objects
stops abduction of neonates as well as demented patients wondering to patients where it should not be
even for equipment that should not be outside of the hospital
Also for surgeries that allows you to know where you are moving from room to room
What is an automated medication dispensing machine? Pros/cons
Contains a variety of med doses that are elctronically dispensed
Prevents drug diversion
increases safety
there can be errors
can give a wrong dose
think radonda vaught
Electronic materials management
Not used by providers that often
monitors how much material is used and how often
used for stocking purposes
pings for expiration
used to reduce delays and false charges
Some people’s entire jobs are to manage this
Hub vs spoke sites for
Hub = site of the provider
spoke = patient
What are some remote patient monitoring?
vitals
glucometer
What is one of the MC types of telemdeicine?
teleradiology
sending images to radiologists
NEEDS to be on US soil!
Risks of telemedicine
higher risk of error
info can be exposed/lost
