GOLD staging for COPD
COPD has progressive airflow limitation characterized by FEV1/FVC < 70%. Spirometry is essential for COPD staging. Global Initiative for Obstructive Lung Disease (GOLD) criteria are used for staging purposes as follows:
GOLD 0 – Normal spirometry, just symptoms = at risk
GOLD 1 – FEV1 >80% predicted = mild
GOLD 2 – 50% to < 80% predicted FEV1= moderate
GOLD 3 – 30% to <50% predicted FEV1 = severe
GOLD 4 – FEV1 < 30% predicted, or FEV1 < 50% normal with chronic respiratory failure
present = very severe
Treatment for COPD includes short and long acting anticholinergics, which improve symptoms and airflow while decreasing hyperinflation in COPD. Long acting beta-agonists can be used as isolated agents for COPD. Smoking cessation is critical and returns rate of loss of lung function almost to normal. Supplemental oxygen decreases mortality.
