What is respiratory distress?
Tachypnea, grunting, nasal flaring, retractions, cyanosis
How do you evaluate a infant for respiratory distress?
History/physical
Glucose
CBC
CXR
What is transient tachypnea of the newborn?
Failure of fetal lung fluid to be reabsorbed at the time of delivery
Hormonal changes associated with labor cause a reversal of Na channels in lung tissue
Failure of channel reversal causes increased respiratory effort to maintain adequate gas exchange
What is TTN associated with?
C section
Perinatal asphyxia
Umbilical cord prolapse
Depressed newborn
What is respiratory distress syndrome?
Surfactant deficiency
Leads to alveolar collpase and protein accumulation
hylaine membrane formation and impaired gas exchange
What is the treatment for RDS?
Natrual surfatant
Artificial surfactant - worse than natural because it lacks protein
NCPAP/Mechanical Ventilation
What are the risk factors for congenital pneumonia?
Prolonged rupture of the membranes >12 hours
Maternal fever
Uterine tenderness
Foul smelling amniotic fluid
