  • Grapefruit
  • <3
  • Protease inhibitors (PI), especially ritonavir (Norvir)
  • Azole antifungals (fluconazole, itraconazole, ketoconazole, posaconazole (Noxafil), voriconazole, and isavuconazonium (Cresemba))
  • Cyclosporine, cimetidine (Tagamet), cobicistat
  • Macrolides (clarithromycin (Biaxin) and erythromycin, but not azithromycin)
  • Amiodarone (and dronedarone (Multaq))
  • Non-DHP CCBs (diltiazem and verapamil)
