Abrupt onset
Fit vascular territories
Negative symptoms
ACA infarct
Predominantly the contralateral lower limb
Left MCA infacrt
Dysphasia, right sided weakness/ numbness
Right MCA infarct
Neglect, left sided weakness/ numbness
Brainstem infarct
May involve diplopia, visual field defect, facial weakness, contralateral limb weakness/ numbness, incoordination
UMN facial weakness
Upper face spared due to bilateral innervation
Bells palsy
Unilateral LMN facial weakness
Ischaemic
Clot in heart, thrown up to brain
Clot in caortid bifurcation
Atherosclerosis within the vessel
Haemorrhage
Surge in blood pressure
Aneurysm burst
Arteriovenous malformation
First test to decide if clot or bleed
CT scan
Critical ischaemia
High metabolic demand of brain- no glucose stores
<20ml/100g/min- electrical function stops (neurones still alive)
<10ml/100g/min- neuronal death, irreversible ischaemia
MRI
Diffusion weighted sequence
Shows ischaemia in 20 minutes

Hypertension
Hyperlipidaemia
Tests for aetiology
Standard ECG
Echo
Ultrasound of carotid artery
Virchov’s triad
Hypercoagulable state
