A
a subjective statement made by a patient describing the most significant or serious symptoms or signs of illness or dysfunction that caused him or her to seek dental care
- always written in the patients own words

what is a medical history
A
- the patients past and present which may contain relevant information bearing on their health past, present and future
- medical history being an account of all medical events and problems a person has experienced is an important tool in the management of the patient
