A

Presenting condition (if symptoms have changed)

Plan for visit + details of treatment provided

  • LA
  • RD
  • Pre-op Radiographs
  • Record tooth was accessed
  • Estimated + Calculated working lengths + PA used
  • Instrumentation completed + MAF Size
  • Irrigants used (usually NaOH and EDTA)
  • Dressing/medicament (usually calcium hydroxide)
  • Temp restoration usually Cavit and GIC
