A

Plan for visit + details of treatment provided
- LA
- RD
- Pre-op Radiographs
- Record tooth was accessed
- Estimated + Calculated working lengths + PA used
- Instrumentation completed + MAF Size
- Irrigants used (usually NaOH and EDTA)
- Dressing/medicament (usually calcium hydroxide)
- Temp restoration usually Cavit and GIC
