What are the 4 steps of the diagnostic process for CMS?
- Obtain some info from the patient:
-history taking, pt centred interviewing - Consider what might be going on & what info you still need to gather:
-illness scripts, differential diagnosis, probability, evidence, testing threshold, tx thresholds - Get this information:
-history taking, pt entered interviewing, physical exams - Take appropriate action:
-testing thresholds, tx thresholds, probability evidence

What doe history taking include?
Asking to about:
-different aspects of pts presenting concerns, current meds, past medical history, family history, social history (diet, relationships, occupation)
What is a symptom?
A manifestation of a disease reported by the patient
Where does history taking get charted?
Subjective notes
What is patient centred interviewing?
Focuses on patients ideas & feelings (esp. fears) about their illness & the impact it has on their functioning & expectations.
Where do physical exams get charted?
Objective notes
What is a sign?
A manifestation of a disease that the clinician perceives.
What is a physical exam?
Inspection, auscultation, percussion, palpation & other maneuvers to gather further info
What is a differential diagnosis?
A list of conditions that are candidates for explaining the patients concern.
What is premature closure?
Failing to consider alternatives after an initial diagnosis is made.
What are disease illness scripts used for?
to compare & contrast conditions
What are patient illness scripts used for?
to facilitate a differential diagnosis
What is evidence? How is it gathered?
- Information that helps you update your estimate of probabilities.
