Intraoperative Monitoring Flashcards
1
Q
Name 2 forms of monitoring?
Specify components of each?
A
- Machine monitors:
Oxygen supply an d pressure in supply lines
A/w pressure
% oxygen in final gas mixture
Must be checked and calibrated everday - Patient monitors:
Clinical observations of vital function
Complicated direct monitors of vital functions
- invasine/ non invasive monitoring
2
Q
How will you clinically monitor depth of anesthesia?
A
Inspecting pupils at regular intervals
3
Q
Clinical observations for depth of anesthesia ( guedel) - 4 stages:
A
- Anesthesia to LOC
- LOC to automatic breathing onset
- Aut breathing to resp paralysis:
- Aut breathing - cessation of eyeball mvmnt
- Eyeball to start of intercostal paralysis
- To completion of intercostal paralysis
+ cess. Eyeball mvmnt; equal pupils;
straight ahead - To diaphragmatic paralysis
- From diaphragmatic paralysis to apnoea and death NO reflex and pupils dilated widely.
4
Q
Which Geudel stages are compatible with surgery
A
Stage 3 level 2&3
Not stage 4
5
Q
Name the special monitoring techniques in anesthetics? 8
A
Blood pressure measurement -automated / direct intra-arterial pressure ECG Peripheral pulse monitor and oximeter Capnography CVP Peripheral nerve stimulation BIS index Temperature monitoring
6
Q
What does the BP measurement require?
Sizes incl.
A
Imflatable cuff with. (14cm adult) - upper arms Pressure gauge (spygomanometer)
NB- diametre of upper arm + 20%
Differ in babies and in leg
7
Q
3 components of balanced anesthesia?
A
Analgesia
Mucle relaxants
Hypnosis (consciousness)
Middle: homeostasis/life
