Intracranial bleeds Flashcards
Location of extradural haemorrhage, cause and CT scan appearance?
Between the skull and dura mater
Caused by rupture of the middle meningeal artery in the temporoparietal region- can be associated with a fracture of the temporal bone
CT scan- bi-convex shape and limited by cranial structures
Typical history- young boy with traumatic head injury and an ongoing headache who is initially ok and then has rapid decline over hours
Location of subdural haemorrhage, cause and CT scan appearance?
Occurs between the dura mater and arachnoid mater
Caused by the bridging veins in the outermost meningeal layer
CT scan- crescent shape and not limited by cranial structures
May occur in elderly and alcoholic patients who have more atrophy in their brains
Presents as:
* Fluctuations in levels of consciousness
* One sided headache
* Confusion
* Unilateral dilated pupil
* Weakness
Location of intracerebral haemorrhage, cause and CT scan appearance?
Involves bleeding into the brain tissue
Presnets similar to a stroke and can occur anywhere in the brain tissue
Location of subarachnoid haemorrhage, cause and CT scan appearance?
Involves bleeding into the subarachnoid space between pia mater and arachnoid mater
Usually result of ruprutres cerebral aneurysm
Prenents as sudden onset occipital headache worst they’ve ever had- thunderclap description
Management of intracranial bleeds?
CT head
Bloods for platelets and coag screen
Scpecialist input
Surgery:
Craniotomy or burr holes
