Q

If you have a TSA + subscapularis repair, be careful of which motion?

A

ER ROM
(IR subscap tendon delayed strengthening to let it heal –> eventually will need to activate w/o pec and teres dominating)

13
Q

T or F: you probably will not get full 180 with rTSA

A

true
*deltoid is doing all the work bc rotator cuff is shot, thats why humeral head is concave and glenoid is convex

15
Q

precautions for TSA

A
  1. no extension
  2. no anterior capsule stretch (sleep supine with pillow under elbow)
  3. no behind back (ext + IR), no excessive stretching especially into ER
16
Q

criteria for progression to TSA phase 2

A

tolerates PROM
achieves PROM in:
90 FLEX
90 ABD
45 ER in scaption
70 IR in scaption

17
Q

what can you start doing in phase 2 for TSA?

A

4-6 weeks post op
restore full PROM
begin AAROM –> AROM

*shoulder submax isometrics in neutral
*horizontal add AAROM

18
Q

how heavy should you be lifting in TSA phase 2? 4-6 weeks post op

A

nothing heavier than coffee cup

19
Q

What are criteria to progress to TSA phase 3

A

tolerates PROM/AAROM/isometrics
PROM: achieves
-140 FLEX
-120 ABD
-60 ER in scap plane
-70 IR in scap plane

-actively elevates shoulder against gravity with good mechanics to 100 degrees

20
Q

when does TSA phase 3 start?

A

at least 6 weeks since post op
GOAL: gradual strengthening/endurance, NM control, gradual return to functional activities

light functional activities

21
Q

when can you progress to AROM and initiate ASSISTED behind back motion following TSA?

A

phase 3: moderate strengthening
weeks 6-?

22
Q

how heavy can you lift in TSA phase 3? (week 6–>)

A

6 lbs limit
*can start assisted behind back motion
can start resisted IR/ER in scaption
*light functional activities
*wean from sling fully
*supine elevation strengthening, progress to standing

23
Q

what are criteria to progress to TSA phase 4?

A

tolerates AAROM/AROM
Flx to 140
ABD to 120
ER to 60
IR to 70 deg
Actively elevate shoulder against gravity with good mechanics to 120 degrees

24
What coupled motion should you still be careful with during TSA phase 4 (advanced strengthening phase)
stress anterior capsule, no combined ER + ABD above 80
25
goals for advanced strengthening TSA phase 4
AROM functional use more advanced function progress WB exercises as appropriate late stage: hobbies, sports
26
with a type 2 slap repair, what should you be careful of during phase 1: weeks 1-6
CAREFUL WITH ER (stresses anterior-inferior capsule --> just repaired) PROM to 10-15 only NO BICEP CONTRACTION can do flexion AROM to 60 can do passive/gentle AAROM wear sling at all times for 4 weeks
27
What should you do during phase 2 for type 2 slap repair?
weeks 6-12 *gradual full PROM/AROM *isotonic strengthening, but no bicep strengthening!
28
when can you start doing resisted bicep/supination exercises following type 2 slap repair?
weeks 12-20 phase 3: minimal protection phase
29
Criteria to progress to phase IV: type 2 slap repair
Full, non-painful AROM Muscular strength of 75-80% of contralateral side
30
phase 4: weeks _ to _ phase 5: months _ to _ slap type 2 repair
phase 4: adv strengthening weeks 20-26 phase 5: Return to sport months 6-9
31
review cervical fusion stuff
phase 1: first 0-4/6 weeks --> in c collar 6-12 weeks usually patients come weeks 8-10 for IE scars close 6-8 weeks phase 2: 4-6 weeks --> pain control, wound healing, c-collar...work on aerobic conditioning *no lifting 10+ lbs, *core, chin tucks, shoulder stability, balance
32
T or F: avoid lifting, push/pulling > 20 lbs up to 3 months post op cervical fusion
True *avoid overhead lifting, end range neck mvmt, heavy stuff
33
when can cervical fusion patients lift overhead/reach overhead
8 weeks *avoid lifting >20 lbs up to 2 months post op *avoid cervical AROM or prone exercises *avoid running/horse riding for 6 months
34
review cervical fusion phase 3 and 4
phase 3: week 6- month 3 Achieve functional shoulder AROM (avoid overhead reaching until 8 weeks) Improve UE strength (<20 lbs first 12 weeks) Gentle cervical AROM until 3 months Good posture phase 4: 3-6 months Return to baseline standing/walking duration and distance - Discharge cervical collar Increase weight limit by 5 lbs every other week as tolerable May begin overhead activities (progress slowly) May initiate elliptical training Hold run/jog/horseback riding/contact sports until 6 months
