Antacids:
increase absorption of enteric coated drugs
affect absorption of tetracycline, PCN, ketoconazole, digoxin
oral anticoags (warfarin)
highly plasma bound
interact with aspirin and sulfonamides
also: phenobarbital macrolides (erythromycin and others) azole antifungals grapefruit juice st john's wort ginseng inhibited by azathioprine (immunosuppressive)
phenobarbital:
affects hepatic clearance of medication
interacts with warfarin
st Jon’s wort
oral contraceptives
cyclosporine
Coumadin
PIs (HIV meds)
Metronidazole:
lithium (renal toxicity)
coumadin (bleeding)
azoles, macrolides, grapefruit juice
digoxin (bradycardia) cyclosporine (extreme immune suppression and nephrotoxicity) coumadin (bleeding) prednisone (steroid toxicity) theophylline (tachyarrhythmia) carbamazepine (CNS effects) trefinadine (tachycardia)
Tetracycline:
digoxin toxicity
also remember antacids
Rifampin
oral contraceptives
cyp450 stimulator ==> fast turnaround for estrogen and progesterone
Benazepril: ACE inhibitor
immunosuppressives
at risk for orthostatic hypotension
propanolol: beta blocker
prednisone
aspirin
levothyroxine
prozac
atorvastatin: cholesterol
stays longer in system if pt on cyp450 inhibitors like HIV meds, erythromycin, ketoconazole
increases digoxin plasma concentration
contraindicated for pts with liver failure and recent TIAs
omeprazole: antacid
rifampin, st john’s wort can reduce its effects
reduces effects of tetracycline, PCNs, ketoconazole, digoxin
contraindicated for pts on NNRTIs
Levothyroxine: thyroid
statins
HTN meds
metformin
clarithromycin (macrolide)
fluconazole
ketoconazole
lovastatin
Hydrochlorothiazide: diuretic. HTN
Corticosteroids NSAIDs, HTN drugs statins DM drugs
contraindicated in pts with sulfonamide allergy
