Correction insulin –

TDD - Total Daily Dose

ISF - Insulin Sensitivity factor

A

ISF = 1500 / TDD

1 unit of novolog will decrease ISF

TDD of a patient is 50

1500 / 50 = 30

Correction will look like this

  • < 150 no insulin
  • 150150-179 1 unit extra
  • 180-209 2 units
  • 210-239 3 units
  • 240-269 4 units

ICR = Insulin to Carb ratio

A

ICR = 450 /TDD = (1 unit insulin per gram Carb)

One time correction = Glucose - 150 / ISF

Starting Insulin in DM Type 1

A

Starting Insulin in DM Type 1 Depends on various factors; coming off an insulin drip or not Usually:

  • ( body wt in kg ) x ( 0.5 ) = TDD of insulin
  • (100 kg) * .5 = 50 TDD

Basal amount

  • ( TDD ) x ( 0.5 ) = starting Lantus dose
  • 50 * .5 = 25 units of lantus

Correction amount

  • 1500 / TDD = ISF (insulin sensitivity factor) to setup novolog correction
  • 1500 / 50 = 10 (everu 1unit to correct 10 Glucose levels)

Novolog with meals ICR

  • 450 / TDD = ICR (carb ratio) to set Novolog mealtime insulin
  • 450 / 50 = Insuline to carb ratio of 9

it is 5 X more concentrated.

A

TDD = 220

Convert to U 500 = 220 / 5 = 44

Will reduce to dose by 30%

44* 0.70 = 30 units U 500 TDD
