Adenosine (Adenocard)
Cardiovascular Agent
Duration: Seconds to minutes and the site is the antecubital area via large bore IV.
Diphenhydramine (Benadryl)
Anti-histamine blocker that works on H-1 receptor site
Used for allergic reactions, hives, itching, rhinitis
Duration: 4-6 hours
IM: 25-50 mg, 25 1.5” needle, large deep muscle
IV: 25 mg diluted over 2 min
Dexamethasone (Decadron)
Glucocorticoid steroid that decreases inflammation
Used for inflammation, allergies, septic shock
IM: 5-10 mg, 25G 1.5” needle, large deep muscle (Glute)
IV: up to 10 mg
Avoid in diabetics
Diltiazem (Cardizem)
Slow electrical conduction of heart and normalize heart rhythm. Used treatment of rapid atrial fibrillation.
IV: 15-20 mg
Epinephrine
Used for acute asthma attacks, croup, allergic reactions (anaphylaxis) and cardiac arrest.
.3 mg, 25G 1 1/2”
Skin over triceps
Contraindication: tachycardia
Famotidine (Pepcid)
Short term treatment of duodenal ulcer, gastritis, allergic reactions
Duration 10-12 hours with 10-20mg diluted over 2 min
Contraindicated in renal disease
Furosemide (Lasix)
Treatment of edema associated with CHF used for management of hypertension
IM: 20-40 mg in 1 or more doses
25G 1 1/2” needle in glute
IV: 20-40 mg
Humulin (R) Insulin
Treatment for diabetes mellitus
8-12 units per dose with Insulin Syringe
Contraindications: Allergy to any ingredient in regular insulin
Lovenox
Prevention of DVT which may lead to pulmonary embolism. And used for those with chest pain and MI.
Duration: 24 hours
Dosage: 1mg/kg with prefilled syringe in Abdomen
NOT GIVEN BY IV
Contraindications: Recent epidural
Methlprednisone (Solumedrol)
Anti-inflammatory for acute and chronic inflammatory diseases (skin conditions, ulcerative colitis, arthritis, lupus)
Duration: 6-8 hours
IM: 125 mg, 25G 1 1/2” needle in glute
IV: 10-250 mg undiluted over 2 min
Contraindications: systemic fungal infections and avoid in diabetics
Metaprolol (Lopressor)
Reduces heart rate and is useful in treating abnormal heart rhythms lowering BP and is helpful in treating heart pain.
PO: 50 mg
IV: 5 mg undiluted over 2 min
Contraindication: cardiogenic shock
Reglan (Metoclopramide)
Prevention of Nausea and vomiting, gastroparesis, migraine, headache
Duration: 1-2 hours
IM: 10 mg, 25G 1 1/2” in glute
IV: 5-1- mg diluted with 10cc of NS over 2 min
Rocephin (Ceftriaxone)
Antibiotic used for pulmonary, skin, sinus, and UTI.
Duration: 12-24 hours
IM: 250 mg mixed with .9ml 1% lidocaine, 25G 1 1/2” glute only
1000mg mixed with 2.1ml 1% lidocaine in glute only
IV: 250 and 1000 mg given over 2 minutes
Contraindications: allergy to cephalosporins (Keflex, Omnicef)
Tetanus and Diptheria Booster
IN urgent care used for patients with wound and puncture injuries and not given by IV.
IM: .5 ml, 25G 1 1/2” in delt
Toradol (Ketorolac)
Pain management
Duration: 6-8 hours
Given IM (60 mg) and IV (30 mg)
Large deep muscle and over 2 min respectively
Contraindications: Aspirin, severe liver or kidney disease, peptic ulcer disease
