Inherited cardiac disease Flashcards
What do genes do?
Make proteins
What can gene mutations lead to?
abnormalities of the protein for which they code
What is cardiomyopathy?
primary heart muscle disease – often genetic
What is hypertrophic cardiomyopathy caused by?
sarcomeric protein gene mutations
Who does HCM affect?
1 in 500 people
What does HCM refer to?
otherwise unexplained primary cardiac hypertrophy
What can HCM cause
angina, dyspnoea, palpitations, dizzy spells or syncope
LVOT obstruction
What is dilated cardiomyopathy caused by?
cytoskeletal gene mutations
What does DCM usually present with?
heart failure symptoms
What does DCM cause?
LV/RV or 4 chamber dilatation and dysfunction
Arrhythmogenic cardiomyopathy (ARVC/ALVC) is caused by what?
desmosome gene mutations
What is the main feature of arrhythmogenic cardiomyopathy?
Arrhythmia
What can ventricular tachycardia indicate in young patients?
Sudden death
All cardiomyopathies carry what?
An increased risk
What is inherited arrhythmia (channelopathy) caused by?
ion channel protein gene mutations
What channels do these channel protein gene mutations refer to?
sodium, potassium, calcium channels
Examples of channelopathies?
long QT, short QT, Brugada and CPVT
What do channelopathies present with?
Structurally normal heart
Recurrent syncope
Why is being aware of QT prolonging drugs important?
Kill people with long QT syndrome
What is sudden cardiac death in young people usually caused by?
Inherited conditions - most likely cardiomyopathy or ion channelopathy
What does Sudden arrhythmic death syndrome (SADS) usually refer to?
normal heart/arrhythmia
What is Familial hypercholesterolaemia (FH) ?
inherited abnormality of cholesterol metabolism
What does FH lead to?
serious premature coronary and other vascular disease
Examples of aortovascular syndromes
Marfan, Loeys-Dietz, vascular Ehler Danlos (EDS)
ICCs are usually what type of inheritance?
dominantly inherited – offspring have 50% risk of inheritance
What are some life saving treatments?
ICD, beta-blockers, statins, vascular surgery
