Isoflurane (MAC, vapor pressure, blood gas partition coefficient, MOA)
A
MOA: halogenated methyl ethyl ether (isolated Cl-), most potent of volatile agents, but slower onset and recovery time MAC: 1.2 BP: 49 degrees C Vapor pressure: 238 mmHg Blood gas partition coefficient: 1.46 Oil gas coefficient: 91 Chemical Structure: "isolated chloride"
Extras:
▾CV: Minimal cardiac depression; preserves carotid baroreceptors, Dilates coronary arteries; concern for ‘reverse-Robin Hood’ (Only happens in hypotensive patients! Will be negligible or not happen at all if you maintain VS’s)
▾Respiratory: Pungent; not used for inhalational induction, Tachypnea less pronounced
▾ Most consistent agent
▾ Could use 2nd gas effect – N2O (insolubility of nitrous oxide will help speed up the process)
