Features of acute mastitis
A
- acute inflammation of the breast
- common in lactating breast - microscopic fissures allow for infection

Pathogenesis of acute mastitis
A
- cracked/inflamed nipple permits entry of organism
- proliferation of microbe (staph) in stagnant milk
- acute inflam, neutrophils - redness, engorgement, swelling, inability to breastfeed, pain
- acute abscess formation, local inflam, tissue destruction

Treatment of acute mastitis
A
- I&D - abscesses
- antibiotics
- excision if not responsive to antibiotics
- if untreated - can drain to surface, forming fistula

Features of idiopathic granulomatous mastitis
A
- rare inflammatory condition
- unknown etiology, hypothesis: autoimmune reaction to secretions from terminal ducts
- lobulocentric granulomatous inflammation w painful scarring

Treatment of idiopathic granulomatous mastitis
A
- DDx malignancy, TB
- steroids - suppress inflammation
- immunosuppressives
- surgery - total mastectomy

Types of breast implants/injections
A
- paraffin injections
- implants filled w silicon gel
- autologous tissue (own muscle)

Effects of breast implants/injections
A
- paraffinoma - reaction secondary to paraffin, multinucleate giant cells
- fibrosis around implant
- foreign body inflammation reaction to leakages (implants can leak into surrounding tissue)
