Q

ACUTE - Contraindication + Treatment goals + treatment + home care

A

CI
1. No lymphatic or circulatory strokes on the lesion and distal to the injury
2. Do not directly treat protective muscle spasms in acute stage

Treatment goals 1. Promote relaxation to reduce pain 2. Reduce swelling to reduce pain
Treatment 1. Massage unaffected area 2. Elevate, cool hydro 3. Lymphatic drainage proximal to injury
Home care: limit active movement due to local pain, swelling, mm guarding

Q

EARLY SUBACUTE - Contraindication + Treatment goals + treatment + home care

A

CI: pressure of techniques directed towards the injury
Treatment goals
1. Reduce edema, pain, swelling, inflammation
2. Reduce sympathetic nervous system firing to promote relaxation
3. Increase local circulation, reduce spasm unaffected areas to remove irritating metabolites, increase tissue health
4. Maintain ROM and strength unaffected side
5. Maintain joint health and joint range affected side
Treatment
1. Elevate affected area
2. Cool/ Mild contrast
3. Lymphatic drainage proximal + edges of injury site
4. Swedish unaffected areas
5. Swedish proximal tissue
6. Trigger point relieve
7. Reduce spasm protocol
8. Passive movement full range
9. Slowly moved a few degrees to mid-range of pain free motion
10. Passive movement full range
Slowly moved a few degrees to mid-range of pain free motion
Home care:
1. Pain-free active movement
2. Isometric contractions the affected mm
3. If inflammation increases because of overdoing –> the exercise should be made less vigorous.

Q

LATE SUBACUTE - Treatment goals + treatment + home care

A

Treatment goals
1. Decrease remaining edema to increase tissue health
2. Reduce trigger points to proximal and local tissue
3. Break down adhesions to promote relaxation
4. Help the collagen fibres align more functionally
5. Promote the removal of debris and break out the adhesions
6. Reduce Pain, reduce hyperemia after friction
7. Increase ROM affected joint
8. Prevent adhesions affected joints, mobilize hypomobile joints

Treatment 1. Swedish over the entire affected limb 2. TP relieve techniques S-bowing, C-bowing, skin rolling, mm stripping, picking up, friction 3. Prolonged stretch 4. Eff 5. Cool , cold 6. Passive ROM mid-range to full range 7. Joint play
Home care: Stretching, strengthening, passive stretch or PIR, full strength isometric contraction to isokinetic exercises Discomfort last > 4h + swelling --> exercise is too vigorous or too quickly

Q

Chronic - Treatment goals + treatment + home care

A

Treatment goals
1. Reduce adhesions to increase tissue health
2. Help the collagen fibres align more functionally
3. Promote the removal of debris and break out the adhesions
4. Address trigger points, hypertonicity and fascial restriction
5. Prevent adhesions affected joints, mobilize hypomobile joints
6. Reduce Pain, reduce hyperemia after friction

Treatment 1. Contrast/ or hot 2. S-bowing, C-bowing, skin rolling, picking up, friction 3. mm stripping 4. Prolonged stretch 5. Eff 6. tp relieve techniques, fascial release techniques 7. Joint play 8. Cool, cold
Home care: 1. Stretching, strengthening 2. Endurance and cardiovascular exercise are encouraged
