Diagnosis using blood tests:
what is required with blood cultures?
rheumatoid factor?
Blood cultures: do three sets 12h apart, 90% diagnosed from 2 +ve sets.
Rheumatoid factor +ve.
Tx of IE
- blind therapy?
- if penicillin allergic?
- Staphs + native valve:
- Blind therapy (native or prosthetic valve): ampicillin, flucloxacillin and gentamycin.
If penicillin allergic then give vancomycin + gentamycin.
flucloxacillin for >4 weeks. (if allergic then give vancomycin).
A 39-year-old female intravenous drug user (IVDU) is admitted to the emergency department with shortness of breath. On examination, she has a temperature of 39.5ºC and is found to have Roth spots, Janeway lesions, and a new heart murmur.

A 62 year old woman is admitted to the medical ward with a 3 week history of fevers and lethargy.
