What is infective endocarditis?
infection producing vegetations on the endocardium usually including the heart valves
In IE more common in men or women

Elderlu- more than 50% of cases occur in those over 60
What are the most commonly valves infected
Mitral (most common), Aortic
What is the most common cause of IE
Staph aureus or strep viridins
What type of IE do immunocompromised and IV drug users get
Fungi
Risk factors of IE
Prosthetic heart valve Structural heart disease Poor dentition IV drug user Pregnant Diabetes
Presentation when infected with staff aureus
Acute bacterial, severe illness progresses over days to weeks
Presentation with streptococci
subacute bacterial, mild-moderate that progresses slowly over weeks to months
When Might you see endocarditis infecting the tricuspid valve
Right sided endocarditis- IV drug users
What percentage of prosthetic valve patients get IE within the first year
1-2%
Clinical manifestations
Fever of unk origin Weight loss New cardiac murmur Petechia Splinter hemorrhages Roth spots Osler nodes Janeway lesions Night sweats Fatigue
If the vegitation dislodges where will it go
Septic embolism- heart, brain, kidney, spleen, liver and extremitis
Diagnosis of IE
Min 3 blood cultures
TEE- 50-80% have vegetation
What is the duke criteria
Uses minor and major criteria
