Risk factors for CAP?

Population with highest prevalence?

A
  • COPD, chronic bronchitis, CV disease, DM
  • Smokers (invasive pneumococcal infections)
  • Alcoholism, neurologic disease (aspiration )
    # Most common in elderly
    # 60% of hospitalizations are in pts >65

CAP pathogens?

A
  • Strep pneumo most common (also most frequent cause of bacteremic PNA from community)
  • H flu, Moraxella
  • GNR less common (enterics in chronic liver disease, alcoholism)
  • Klebsiella = alcoholism
  • Pseudomonas = structural lung disease, chronic steroids
  • Staph aureus = think if CAP follows influenza, cavitary PNA, IVDU, recent MRSA skin/soft tissue infection
