Q

Factors affecting drug
-prostatic fluid + tissue penetration

A

Prostatic fluid penetration
-Lipid solubility
-Degree of ionization in plasma (pKa)
-In case of Water solubility: molecular size + shape

Tissue concentration
-plasma levels related to dose and bioavailability
-drug related:
protein binding qualities
Concentration gradient
Lipid solubility
Plasma half life

-tissue factors: inflammation
Vascularisation
Capillary porosity
(acute inflammation permits better penetration vs chronic infection with fibrosis and scarring)

Q

Why are fluoroquinolones the ideal prostate antibiotic in terms of pharmacokinetics

A

Small molecular size
High lipid solubility
Low protein binding
Zwitterions(+ve&-ve charged groups)

Excellent prostatic penetration (0.9-2.3xgreater than serum concentration)

Q

Describe 3 zones of renal hydatid cysts

A

Peripheral : host fibroblasts that may calcify
Intermediate laminated: becomes hyalinised
Germinal:nucleated epithelium gives rise to brood capsules and daughter cysts
