What are the indications for extraction
unrestorable teeth symptomatic partially unerupted teeth traumatic position orthodontic indication interference with construction of dentures
What is the periotome
it is a mini blade that is worked down the PDl space and it cuts the PDL ligament so that the tooth can easily come out
What are the 3 basic modes of action for the mechanical principles for tooth elevation
wheel and axle
lever
wedge
What is the wheel and axle motion
Placed elevator into gap from buccal side so its almost horizontal
Wedge it in so tip is engaging acj or as deep u can possibly go
Then rotate the instrument, rotates on an axle
And as u rotate it twists upwards and as it does that it elevates the tooth
What is the lever motion
generates more force
can be dangerous
you rest it on bone and wedge it against the surface of the tooth and move it up using a lever motion
can fracture mandible
What is the wedge motion
can be done with a luxator
it is pointy and sharp
the tip is pushed into the PDL space and the root just comes up
usually need a sharp instrument
What are the various points of application for elevators
mesial application buccal application distal application superior application mesial then buccal application medial and distal application changing a point of application of force from distal to mesial inferior application
Describe mesial application
usually start with medial application
the concave surface faces the medial surface you want to elevate and you do a rotation action which forces the tooth upwards and backwards which for a lower 8 is good if there is space
Describe buccal application
if it doesn’t come out then you can swap to the buccal side and keep using the elevator and put it into the furcation and push it up and lingual this time. this only works well in the region of the lower 8 as this is where the bone is thin
Describe distal application
unusual because there is usually teeth in the way and hard to get your elevator all the way back there, would have to stretch the cheek
Describe superior application
would have to remove all of buccal bone
raise a flap and drill it all away and put the tip in and push it down
unusual but put in the books
Describe mesial and buccal application
do medial then buccal
combination in sequence
works well for wised teeth
Describe medial and distal application
not recommended
used exactly the same time
generate a vertical force
going to end up fracturing their mandible as no hand is supporting the jaw
