Weakness of the bladder outlet:
- external sphincter
- pelvic floor muscles
What causes urge incontinence?
Detrusor muscle instability, ie. overactive bladder
- UMS lesion
- detrusor muscle disorder
What symptoms (besides urgency) are associated with urge incontinence?
Frequency (but low volume)
Nocturnal incontinence
What causes overflow incontinence?
Urinary tract obstruction
- prostatic hypertrophy
- prostate tumour
What symptoms are associated with overflow incontinence?
hesitancy
reduced stream
post-micturition dribbling
Describe some general treatment strategies for incontinence
Weight loss (decrease intra-abdominal pressure) Reduce bladder irritants - caffeine - fruit juice - alcohol Fluid control - volume - type e.g. reduce caffeine
What is the main treatment for stress incontinence?
Pelvic floor exercises
What treatment options are available for urge incontinence?
Bladder retraining
Anticholinergics e.g. oxybutynin
Botulinum toxin
Mirabegron
What treatment options are available for overflow incontinence?
Treatments for prostatic hypertrophy:
- alpha blockers e.g. tamsulasin, dexazocin
- finasteride
Catheter (last resort)
