What is stress incontinence
Incontinence that occurs with increased abdominal pressure: such as coughing, lifting, exercising
What can cause stress incontinence?
Multiparous pregnancies
Radical prostatectomy
What are treatments for stress incontinence in order of preference
- Lifestyle
- Kegel
- Kegel with biofeedback
- Topical vaginal estrogen in women
- Pessaries or surgery
How should Kegel exercises be performed?
- Contract pelvic floor like you are preventing voiding
- Hold to a count of 10
- Sets of ten should be done 3 or 4 times a day
- It can take several months with perfect adherence
What is the detrusor muscle?
It’s the muscular component of bladder that causes ruination when it contracts
What is urge urinary incontinence?
Alan known as overactive bladder and detrusor over activity
What are secondary causes of urge incontinence?
Can be seen with spinal cord lesions and bladder irritants
May be associated with fibromyalgia
What are treatments for urge incontinence?
Weight loss and remove irritants from lifestyle
Kegel exercises are also recommended
Bladder training
Meds
Botox injections
What are bladder irritants that can contribute to incontinence?
Caffeine, tobacco, vitamin c
Alcohol, pepper and other spices may play role
What is overflow incontinence
The bladder retains urine and becomes too full causing spontaneous leakage of urine
What are causes of overflow incontinence?
Bladder outlet obstruction
Meds which reduce detrusor activity
Neuropathy
What is functional incontinence?
The bladder works fine but the person has mobility issues and unable to reach bathroom
What is bladder training
You note the frequency of incontinence then schedule toileting trips at shorter intervals. If urge occurs use Kegel until urge passes then toilet. Slowly lengthen the interval over time.
What types of lifestyle interventions help incontinence?
-weight loss - an 8% drop in weight reduced incontinence by 50%
-avoid bladder irritants - caffeine, vit C, pepper, hot spices, and maybe alcohol
-watch for meds which increase urine flow and high fluid intake, especially at night
- treat constipation which can press on bladder and it’s outlet
