Benefit of RT?

University of Tokushima, Tokushima, Japan (Kondo et al, Ann Thorac Surg, 2003): Stage III-IV thymoma 5-yr OS 93% for total resection, 64% SRT, 36% inoperable
Thymic carcinoma 5-yr OS 67% GTR, 30% STR, 24% inoperable. No benefit to RT in Stage III-IV thymoma or thymic carcinoma, or in GTR stage II-III thymoma.

Japanese Association for Research on the Thymus Database Study (Omasa et al, Cancer 2015): Masaoka Stage II-III thymoma and thymic carcinoma. Stage II-III thymic carcinoma: RT was associated with improved RFS for but not OS. Stage II-III thymoma: RT was not associated with benefit in RFS or OS. No change on subgroup analysis

Fox Chase (Curran et al, JCO, 1988): Thymoma patients s/p surgery. Masaoka stage and extent of resection were correlated with recurrence.
Stage I, R0: no recurrences
Stage II-III,R0: mediastinal recurrence 53% vs. 0% with RT
Stage II-III, R1/R2: recurrence 21% with RT.
R0 alone appears to be inadequate therapy in higher stage thymoma.
