What is the definition of dental implant?
- Artificial tooth root that is surgically anchored into the jaw to hold a replacement tooth or teeth or a denture in place. The benefit of using implants is that they don’t rely on neighbouring teeth for support.
What can an implant restore ( not just dentistry related)?
- A single tooth
- Multiple teeth
- Can secure a denture firmly (implant overdenture)
- Eyes, ears, hearing aids & noses
What is the role of GDP for maintenance of dental implants according to SDCEP guidelines?
- OHI
- Ensure pt can peform optimal plaque removal around implant
- Examine peri-implant tissues for signs pf inflam and BOP and/or suppuration and remove supra and submucosal plaque and calculus deposits and XS residual cement
- Radiographic examination when clinically indicated
Is the BPE important for dental implants?
- Not appropriate
What is Peri-implant mucositis?
- Inflammation of the peri-implant mucosa with no evidence of crestal bone lone
- Tissues will appear red and swollen and may bleed on gentle probing
What is the management of peri-implant mucositis according to SDCEP guidlines?
- Radiographic examination to assess peri-implant bone levels compared to baseline radiograph to exclude peri-implant mucositis
- Treat as previous SDCEP maintence
- Re-assess at future visit that inflammation has settled and stable situation been achieved
What is Peri-implantitis?
- Infection with suppuration and inflammation of the soft tissues surrounding implant
- Clinically significant loss of peri-implant crestal bone after adaptive phase
- Tissues appear red and swollen and may bleed on gentle probing and will be suppuration
What is the management of peri-implantitis according to SDCEP guidelines?
- Radiographic examination to evaluate peri-implant bone levels compared with baseline radiograph
- If clinically sig progressing crestal bone loss detected refer back to clinician who placed implant
- If cant refer then treat as maintenance section and arrange follow up 1-2 months , where no improvement seek advice from secondary care
- If inflammation settled and stable situation achieved, arrange radiograph follow up 6-12 months
What are some common post implant txt complications?
- Peri-implant mucositis
- Peri-implantitis
- Loose/fractured components
- Late implant failure
What is the basic technique for implants?
- Diagnose
- Plan
- Place - Raise flap, place implant, place cover screw, suture
- 3 months and implant is integrated into bone
- Uncover implant
- Place abutment
- Take imp with coping and putty wash
- Take opposing arch impression
- Chose colour
- Place temp
- Cast imp and lab creates cement crown for the implant like any other crown
When you have multiple teeth what are the two options for implants?
- Cement retained
- Screw retained
What do you use to secure a denture firmly when the pt has implants?
- Locator abutments
- Abutment has internal and external component
- Denture clips onto external component in mouth
- Ease of fit
What are some different types of methods for securing a denture firmly?
- Ball abutments
- Locator abutments
- Gold bar (more expensive)
- CAD-CAM titanium bar
- Novaloc abutment (less common)
- Magnetic retention (less common)
