Immunosuppresive Agents : ) Flashcards
What are 3 type of drugs used for prevention of transplant rejection
- Glucocorticoids
- immunophilin ligands
- mycophenolate
what are the 3 types of immunophilin ligands
- cyclosporine
- tacrolimus
- sirolimus
explain the MOA of cyclosporine
cyclosporine is a peptide that binds intracellular cyclophilin to form a complex that inhibits the phosphatase cacineurin
what is cacineurin/what is it needed for
Ca2+ calmodulin activated enzyme. Needed for the activation of NF-AT(transcription factor)
what is NF-AT used for?
required for the expression of IL-2
why is IL-2 needed?
IL-2 is essential for T-cell maturation which are involved in acute transplant rejection
explain the MOA of tacrolimus
- binds to immunophilin FK-binding protein (FKBP)
- this inhibits calcineurin
- this inhibits expression of IL-2
what is the MOA of siromilus
- binds immunophilins and acts similar tocromilus and cyclosporine
- also inhibits B cells
what is a derivative of sirolimus and what is it used for
everolimus: used for cardiac transplantation
what is the MOA of mycophenolate mofetil (MMF, CellCept)
inhibits de novo purine synthesis (guanine nucleotide)
CU of thalidomide
treatment of multiple myeloma
MOA of thalidomide
inhibits angiogenesis and TNF-α. —> has inflammatory activity
S/Es of thalidomide
TERATOGENIC
- Amelia (absence of arm)
- phocomelia (short stumps for limbs)
what type of agent is azathioprine
cytotoxic
mercaptopurine’s prodrug is what?
azathioprine
cyclophosphamide is what type of drug
cytotoxic agent
describe leflunomide (CU, prodrug of…, )
cytotoxic agent : prodrug of pyrimidine biosynthetic inhibitor
-inhibits mitochondrial dihydroorotate dehydrogenase
CU: rheumatoid arthritis
what type of drug is hyroxychloroquine and how does it work
cytotoxic agent : interferes with the antigen presentation on major histocompatability complex (MHC)
