6 mo + annual
<8 yo and 1st dose then 2 doses 4 wks apart
Pneumo
<2 yo need 4 dose (PCV13) @ 2,4,6, and 12-15 mo
> 65 yo need 1 dose (PCV20)
> 65 yo or 19-64 (health cond) then PPSV23/PCV15, 1 dose followed by either of these two prior >1 yr later
Rotavirus (LIVE) - oral drops
age 8 mo or less then 2-3 doses
2 dose = mix
3 dose = rtd
RSV
60 yo+ need 1 dose
Polio (IPV)
<7 yo need 4 dose @ 2,4,6-18 months and then @ 4-6 yrs old
Hep A
age 12 mo then 6-18 months later need 2 doses
2 dose, 6 mo apart
= at risk adults (men who have sex with men (MSM), drugs, chronic conditions
HPV
<15 yo need 2 dose (0 mo dose 1 then 6-12 mo later 2nd dose)
> 15 yo need 3 dose (0,2,6 mo)
preferred age 11-12 yo
27-45 clinical decision
Varicella
chicken pox (LIVE, sc, no pregnant)
- age 12-15 mo 1st dose
- age 4-6 yo 2nd dose
19-65+ , 2 doses (12 yo and less 3 mo apart; 13+ 4-8 wk apart
SHINGLES >50 , 2 dose (2 mo apart eg 0 mo then 2-6+ mo)
DTAP
