define IHDS
group of disorders caused by imbalance between the myocardium’s oxygen demand and supply
what are the causes of Ihd
1- partial or complete narrowing of the coronary arteries; ATHEROSCLEROSIS, vasospasm/ vasculitis or thrombosis
2- myocardium hypertrophy (complicating HTN)- increased o2 demand
3- low Po2- anemia/ lung disease
how does Ihd present
1- clinically silent (<705 occlusion)
2- Angina pectoris
3- Myocardial Infarction
4- chronic IHD
5- Sudden cardiac death
what are the risk factors of IHD
1- Modifiable
-Low physical activity
- high LDL diet
- smoking/ alcoholism
- Diabetes
- hypertension
- Obesity
- Stress
2- Non modifiable
-Genetics
- Age
- sex (more in men)
which coronary arteries are most commonly occluded
Left anterior descending
Right coronary artery
Left circumflex artery
whatre the diff types of angina
1- Stable angina- >70% occlusion
2- Unstable angina- rupture of plaque
3-prinzmetal angina - coronary vasospasm
Stable angina Features
- crushing substernal pain that radiates to the left shoulder and jaw
-On exertion
-Lasts <20 mins
-SOB - Diaphoresis
-ST depression (subendocardial ischemia)
Unstable Angina features
-prolonged/ recurrent substernal pain at rest
- cand progress to Mi If not managed
- ST depression
prinzmetal angina
intermittent chest pain
ST ELEVATION ( due to transmural ischemia)
