Metabolism of ifs
Half life
Choloro aldehyde responsible for neurotoxicity
By liver cytochrome p450 , 3A4/5
T half : 3 to 10 hrs
Dose of ifs
Head and neck : 1g per m2 day 1 to day 3 every 28 days as a part of TIC regimen
NHL: 1g per m2 day 1 to day 2 every 28 days as a part of ICE regimen
Testis : VeIP regimen 1.2 Gy per m2 every 28 days
Sarcoma: 2gm per m3 every D 1to 3 days every 28 days MAID regimen
Side effects of ifs
Myelosuppression
Urotoxicity : due to large quantity of acroelin , secondary bladder cancer
Nause vomitting : moderate risk
Alopecia
Neurotoxicity : chloro aldehyde: seizures, headache, along with aprepitant
SIADH
How to treat neurotoxicity with ifs
Methylene blue
Risk factors:
Decreased albumin
Increases creatinine
Pelvic disease
