What is the quadruple aim?
- Improving the patient experience of care (including quality and satisfaction);
- Improving the health of populations
- Reducing the per capita cost of health care.
- Staff satisfaction
What is a balancing measure?
Determines whether changes designed to improve one part of the system are causing new problems in other parts of the system. For example, does this new QI change improve staff satisfaction but decrease client satisfaction?
What is an outcome measure?
A measure that answers the question “What are the end results of our QI work.”
What is a process measure?
A measure that captures the changes your quality improvement efforts make to the inputs or steps that contribute to system outcomes.
What are the characteristics of good data in QI?
Sufficient to reflect real patterns Relevant to the problem. Representative of actual conditions under study Useful. Cost-effective in the collection process
What are the components of the data collection plan?
- Specify the objective for the measurement
- Identify relevant measures.
- Develop operational procedures for data collection (who, what, when, where, and how)
- Begin data collection
What is random variation?
Variation that arises from factors that are inherently part of the process
What is non-random variation?
A variation that arises from a unique, unpredicted event
What do run charts visulize?
Process performance and variation
Signals of improvement (or not)
Sustainability of improvement efforts (or not)
What is the median?
It is the middle number in series of numbers.
It has the same number of points above and below it
It is the middle data point for an odd numbered data set
It is the average of the middle two data points for an even number of data points.
What is a shift (in a run chart)?
Six or more consecutive points either all above or all below the median.
Skip values that are on the median as they do not make or break a shift.
What is a trend (in a run chart)?
5 points all going up or going down.
If the value of two or more successive points are the same, ignore one of the points when counting. Like values do not make or break a trend.
What is a run (on a run chart)?
A rune is a series of points in a row on one side of the median.
Best approach is to count the number of times that the data lone crosses the median, and add one.
Statistically significant change is signaled by too few or too many runs.
What is an astronomical value (on a run chart)?
A data point that is a blatantly obvious anomalous value.
