Contraindications for NSAIDs
- Asthma
- Allergy to aspirin
- CV disease (HF promote sodium and water retention)
- Renal problem e.g. CKD (risk of AKI & worsen renal fn)
- Liver cirrhosis (risk of variceal bleeding)
- Ascites (promote Na and water retention)
- Elderly with Hx of GI bleeding
- Systemic steroids, anti- coagulants, anti-platelet
- 3rd trimester of pregnancy (20 wks or 24-40 wks)
- Active bleeding/ ulcer
- Concomitant diuretics, ACEi/ARB
Can Losartan and NSAID be given tgt? Why?
No, risk of AKI if have dehydration
What drugs to avoid in NSAID use?
- Lithium (Incr Li by 50%)
- ACEi/ARB
- Systemic steroids, anti-coagulants, antiplatelets
- Diuretics
Signs of AKI
No / reduced urination
What are missing from MAI
- Untreated Indications
- Adverse Drug Reactions
- Failure to receive drug
Signs of MI in elderly
Stomach ache, nausea, confusion
