What diseases are encompassed in IBD
Ulcerative colitis, Crohn’s disease
What is UC
nonspecific, contiguous mucosal inflammation involving the rectum and externding proximally, but limited to colon
CD is what?
focal, deep inflammation of GI mucosa that can affect any part of the GI tract
What is the main symptom of pts with Ulcerative Colitis sxs?
bloody diarrhea, rectal urgency
Chron’s disease sxs?
diarrhea,abdominal pain and weight loss
What IBD involves the colon, which one involves the whole GI? Which one has fistulas?
UC, CD, CD
What is a fistula?
an abnormal connection connecting two parts of the body, usually bowel to bowel but can be bowel to bladder, etc
pathogenesis of IBD
genetics, infectious agent, environmental factors, alterations in the immune system
What gene is connected to IBD?
NOD2; normally assiaciated with monocytes that recognize bacteria (immune system gene)
What is main cytokine involved in IBD?
TNFa
Does surgery cure UC or CD?
no not always; 2/3 of CD pts have post surgery recurrence
What are some non-pharmacologic management?
diet modification, surgery, TPN
How long do AZA and 6-MP take to work?
3-6 months
What are the immunomodulatroy agesnts for UC and CD?
Azathiprine, 6-mercaptopurine, cyclosporine, methotrexate, tacroliums and mycophenolate
When do you use amino salycilates?
1st line for UC and even CD in mild
