What is IBD?
1.IBD is a range conditions that cause inflammation to the gastrointestinal tract.
2.Gastrointestinal disorders cover diseases of the alimentary tract– from oesophagus to anus– associated organs (liver, gall bladder, pancreas).
What are the common disease of the GIT?
1.Gastro-Oesophageal Reflux Disease– 20% of population of US
- Haemorrhoids (75% of population > 45)
- Chronic constipation
4.Gallstones
- Irritable Bowel Syndrome
- Liver disease
7.Pancreatitis
- Ulcerative Colitis
- Crohn’s disease
How is IBS treated?
Taking immunosuppressants
Key information about Crohn’s disease?
1.Can start in mouth/stomach/oesophagus, but seen in colon and small bowel.
- Causes inflammation that penetrates through the mucosa into tissue and muscle.
- Repair causes thickening of bowel and reduction in lumen
4.large areas of DEEP inflammation/necrosis – ‘cobblestoning’.
5.– multi nucleated macrophages surrounding lymphoid cells – diagnosis.
Key information about Ulcerative colitis?
- Begins in distal colon and rectum and proximally moves up colon – progressive.
2.Ulceration and loss of epithelium through necrosis exposing tissue–
What is seen in the histology of Crohn’s disease?
1.Granuloma
2.Bowel thicken
3.Narrow lumen
4.Neuromuscular hyperplasia
5. Fissuring
What is seen in the histology of Ulcerative colitis?
- Crypt distortion
- Diffuse mucosal inflammation
3.Mucosal ulceration
4.Dysplasia
What are the current treatments to reduce?
1.Aspirin- long term
2.Corticosteroids- short term
3.Azathioprine (slow acting)
4.Ciclosporin ( Fast acting)
What cancer are ppl at risk of getting when they have IBD
Colorectal cancer as a result of chronic intestinal inflammation
What does normal, Crohn’s and ulcerative colitis look like
Normal- scope view (Clear no blockage)
Crohn’s - Cobblestoning ( Build up/ thicken walls )
Ulcerative colitis -build up (ulceration and curving mucosa from ulcerative now formed as pseudo polyps)
